is a division of Connecticut Community Care, Inc., (CCCI) a private, not-for-profit care management organization with offices in Connecticut.  For over 25 years, CCCI has served the needs of the elderly and adults with disabilities, and has helped more than 70,000 individuals live safely and independently at home. 

Connecticut Community Care, Inc. (CCCI)

is a private not for profit organization that serves as an Access Agency for elders who meet the age, financial and functional criteria of the publicly funded Connecticut Home Care Program for Elders. CCCI is the largest not-for-profit care management service for elders and individuals with disabilities in Connecticut, providing care management services to older adults and other disabled persons with similar chronic care needs and their caregivers, to assist them in remaining as independent as possible in the most cost-effective manner.
