The Direct Support Professional (DSP) provides direct hand on care and support to individuals with developmental disabilities, assisting them in achieving their personal goals and maximizing their independence. The DSP works as part of an interdisciplinary team to ensure the provision of high-quality, person-centered services.
Essential Duties and Responsibilities:
• Direct Care:
o Provide assistance with daily living activities, including personal care dressing, toileting, meal preparation.
o Assist individuals with community integration activities, such as shopping, working, attending appointments, and participating in recreational activities.
o Implement individualized support plans, including behavioral support plans and person-centered plans.
o Monitor individuals' health and well-being, reporting any concerns to appropriate personnel.
o Ensure the safety and well-being of individuals at all times.
• Communication and Documentation:
o Maintain accurate and timely documentation of all services provided. (Notes must be inputted into Therap within 24hr of last shift).
o Communicate effectively with individuals, families, and other members of the interdisciplinary team.
o Participate in team meetings and staff trainings. All DDS annual and bi-annual trainings must be completed in Relias upon expiration. CPR/FA and PMT must be maintained current.
o Maintain professional and respectful communication at all times.
• Day Program/Home/Community Maintenance:
o Assist with maintaining a clean and safe living environment.
o Assist with household/day program tasks, such as laundry, cleaning, and meal preparation.
o Assist with transportation and travel arrangements.
• Other Duties as Assigned:
o Participate in agency-wide events and activities.
o Assist with emergency situations as needed.
o Perform other duties as assigned by supervisor.
Physical Demands:
• Ability to stand, walk, sit, and bend for extended periods.
• Ability to lift and transfer individuals.
• Ability to reach, grasp, and manipulate objects.
• Ability to communicate effectively both verbally and in writing.
Requirements
Valid drivers license
CPR/First Aid trained
