Client is 44 years old male with Cerebral Palsy, Using a power wheelchair, Client is fully dependent for all major activities of daily living, but has high functioning cognitive ability and is able to direct his own care.

*transferring from bed to wheelchair
*showering
*dressing
*shaving
*brushing teeth
*medications
*meal preparation and feeding
*toileting
*range of motion and stretching assistance is desired
*housekeeping is also needed
